Baylor Scott and White is now providing a COVID-19 screening questionnaire.

The questionnaire is available through MyBSWHealth, accessible via the web at https://my.bswhealth.com, or via mobile app on Apple iOS or Android devices.

Anyone who is experiencing flu-like symptoms or has concerns about their risk of infection with COVID-19 to first take the screening questionnaire prior to scheduling an appointment, walking into a clinic, or arriving at a hospital emergency department or COVID-19 specimen collection site.

Those interested in installing the app can text BETTER to 88408 to have the device sent to their phone.
